Growing Patient: a Rationale Guide to Expanders

Maxillary expansion is a fundamental orthodontic procedure for the treatment of dento-skeletal malocclusions in growing patients. Although the main indication is the crossbite, this method makes it possible to treat a wide range of malocclusions, favoring a balanced growth of the palate by exploiting the bone plasticity typical of this age group. The management of the Rapid Palatal Expanders requires a thorough knowledge of the indications, the techniques available and the specific physiological characteristics of growing patients. A rational approach to orthodontic treatment implies a thorough clinical evaluation, taking into account not only dental abnormalities, but also skeletal growth, the patient's age and individual needs. In this context, it is essential to select the most suitable expander for each case. Recently, the introduction of skeletal anchorage as the point of application of orthopaedic expansion forces has significantly enlarged the therapeutic opportunities offered by the rapid palate expander, allowing malocclusions in adult patients to be treated as well. However, the market offers many rapid palatal expander designs that, unfortunately, are sometimes neither adequately clinically validated nor supported by solid scientific evidence. The aim of this lecture is to provide clear guidance on the choice of the most appropriate rapid expander according to the patient's age, while maintaining a constant focus on predictability and reproducibility of results, all supported by scientific evidence.
